Forms are containers that hold Items and define how those items should be displayed in the player. Forms are used to construct Tests.
Authoring a New Form
Pre-requisite: Items need to exist in an Item Bank in ADAM.
- Navigate in ADAM to Authoring
- Choose Forms from the menu
- Set the Bank. When you create a new form, it will be created in the bank used in the search
- Click the Create New button in the top right of the Forms search page
This takes you into the New Form Creation pages. There are five tabs within the Form you can use these bolded links to navigate in this article to each of the tabs for instructions:
The page defaults to the 'Form Builder' tab.
Form Builder Tab
The Form Builder Tab is where you organize the items for this form. This includes the option to create multiple sections using the Add Section button and loading items into a section using the Add Items button.
Each section has Section Settings to enable: calculator, show section instructions and timers. Additional Form settings are covered in the Form Settings Tab section.

- Form Name - Set the name for this form.
- Form Identifier - This is a system created identifier.
- Form Type - Choose between a Fixed or Adaptive type. If you choose an Adaptive Form, only CAT compatible items can be added.
- Form Status - Status is defined during Bank Setup. Choose the appropriate Form status from the list.
Section Setup

- Section Name - Set the name for this section.
- Section Identifier - Set a section identifier, usually a version of the section name without spaces.

Section Settings
Each section has settings to enable section features. Choose what features are available per section and click the Save Settings button.

- Calculator - choose from the calculator types found in the drop down
- Timer Settings
- Check the Use Timer checkbox to enable the timer and set the timing options
- Timer Limit - How many seconds should be put on the timer
- Warning Time - How many seconds should be remaining on the timer when the timer warning is displayed to the student
- Time Limit Type - A Soft warning does not force the student out of the section when the timer expires. A Hard warning forces the student out of the section when the timer expires.
- Check the Use Timer checkbox to enable the timer and set the timing options
- Show Instructions Screen
- Set the check box to show an instruction page
- Click the Add Screen button to choose from the already created Resources. If you haven't already authored the Resources page you need for instructions, you can add the resource page later.
- click the Add Resource button to save your instruction page.
- Click the Save Settings button

Add Items
Each section needs to have at least one item in it. Click the Add Items button to start adding items to the section.
- Search for items in the bank
- Place a checkmark in front of the items you want to add to this section
- Click the Add Items button to add the items into the section
- Use the dragger bars to re-arrange the items into the desired item order
- Save the changes to your Form by clicking the Save Changes button at the top of the page
Add Section
If you need to add a new section, click the Add Section button repeating all the Section Setup steps above.
Settings Tab
The Settings Tab defines aspects of the entire form. These settings are broken into four sections: Layout and Navigation, Section Navigation, Tools and Accessibility and Accommodations.
- Set the checkbox in front of the features you want available for this form.
- Click the Save Changes button on the top right of the page to save the updates.
Layout and Navigation
Show Title - two additional field 'Display Title' and 'Display Subtitle' are shown. The Title and Subtitle are displayed in the test player on the first page.
Show Student Name - the tester's name is displayed in the test player
Show Focus Mode - the tester has access to this button that expands the player to display the content more prominently
Show Question Number - the question number is displayed in the test player
Show Progress Bar - the progress bar is displayed in the player showing the tester their progress through each section of the test. If enabled, the color of the progress bar can also be set.
Show Item Progress - *
Show Review Screen - a review screen is displayed at the end of each section showing the tester information about the item attemptedness or item flags in the section.
Show Final Review Screen - a review screen is displayed at the end of the test showing the tester information about the item attemptedness or item flags for the test.
Show Pause Button - the pause button is enabled in the test player for the student to pause their test
Warn on Change - if a tester attempts to move to a new question without first attempting the items on the question they are currently on...they will receive a warning that they have not attempted all items
Show Redirect URL - following the test, the tester can be redirected to this specified URL instead of remaining on the 'test submitted' page.
Section Navigation
Block return to section - the tester cannot return to this section after moving on to the next section.
Wait for proctor release - the tester cannot move to the next section until the Proctor 'allows' the tester into the next section. The proctor uses the Proctor Dashboard controls to release the testers into the next section.
Tools and Accessibility
Flag Item - the tester has access to flag items they would like a reminder to revisit. Flagged items are displayed in the Review Screens.
Choice Masking - the tester has access to obscure choices they have determined are not the correct answer. This is a tester strategy to eliminate choices one at a time.
Notepad - the tester has access to a notepad. The notepad is cleared between sections.
Line Reader - the tester has access to obscure the area around the line they are currently reading. This is a tester strategy to eliminate 'noise' by allowing the tester to focus on one line of text at a time. The tester manually moves the line reader to read new lines of text.
Color Scheme - the tester has access to modify the color palate to create either more or less contrast between the font color and background color. This is a tester strategy to make the text easier for the tester to interpret.
Zoom - the tester has access to modify the magnification of the content. This is a tester strategy to make the content more accessible when it is either too large or too small for the tester.
Font Size - the tester has access to modify the font size. This does not affect images or other design elements.
Dictionary - *
Highlighter - the tester has access to highlight text in the content and change the background color by setting a highlight color. This is a strategy for the tester to set highlight reminders within the content.
Accommodations
TTS Accommodations - if text to speech is allowed on this form, two things need to be true. 1) choose an accommodation ADAM should look at to enable TTS 2) only testers with this accommodation type will have TTS enabled in the test player.
Metadata Tab
During the Bank setup of Form Metadata, you determined the 'extra' fields needed to define Forms. The values for this form are set in the Metadata Tab. In this example a single Form Metadata field is set up to collect the Operational Period.
- Set any metadata field data for the form
- Click the Save Changes button on the top right of the page to save the updates.
